OneNote Forms???

I am a tablet PC user. I have found that you can easily create an inkable form in Windows Journal.

Can you create a form from a scanned image and use it as a background to fill out in OneNote? There may be a better way, but I like using OneNote for the organization of the forms. Any suggestions???
