How can I create a form with check box form fields AND text form fields that BOTH allow a user to use the template by entering data in the text form fields and choosing the check box fields as needed. Currently I get either one (protected=check boxes function) or the other (unprotected=text form field functionality)

-Text Form Field
- Check Box Form Field
