Table of contents issue

I have a very large document with headers throughout that link to the table of contents. I have a couple entries that are doubled and I can't figure out or how to remove them. Of course I can highlight and delete but that doesn't help me for the next update.

Some times the column breaks will have the same header as the header does so it was duplicating the entry in the TOC. So insert a hard enter and the problem will be cleared up.
